Verification Forms: Complete these forms only if you have been notified by our office that you have been selected for verification. You can review your file at mybama.ua.edu to see if you are required to complete any of these forms.

Special Circumstance Forms: If there are new or changed circumstances not correctly reflected on your Free Application for Federal Student Aid (FAFSA), review the forms below to see if your circumstance can be considered.

Consortium Agreement: A consortium agreement is a contract between the student, the host institution, and The University of Alabama. This agreement allows The University of Alabama to process Federal Student Aid for the student while taking courses at another university.

Federal Parent Loan for Undergraduate Students (PLUS) Applications: We must receive the results of your completed FAFSA before we can process your Federal PLUS Application. IMPORTANT NOTE: Please read these important changes to 2016-17 Parent PLUS processing before you apply for PLUS loans. Apply for a Parent PLUS loan here after April 15, 2016 for 2016-17.

Federal Graduate PLUS Loan Applications: We must receive the results of your completed FAFSA before we can process your Federal PLUS Application. IMPORTANT NOTE: Please read these important changes to 2016-2017 Grad PLUS processing before you apply for PLUS loans. Apply for a Graduate PLUS loan here after April 15, 2016 for 2016-17.

Parent PLUS Authorization to Credit Account: Use this form to allow your Parent PLUS loan proceeds to pay all charges on the student account.

Request to Change Parent PLUS Loan: Complete this form if you would like to update your current Federal Parent PLUS application.

Graduate PLUS Authorization to Credit Account: Use this form to allow your Graduate PLUS loan proceeds to pay all charges on the student account.

Request to Change Graduate PLUS Loan: Complete this form if you would like to update your current Federal Grad PLUS application.

Waiver of Federal Aid: Complete this form if you do not wish to apply for federal aid but need our office to process another type of aid for you.
